Masters, Cadets, Youth carnival nomination form

Nominations must be received by the cut-off date prior to each carnival. Please complete one form for each competitor.

Please Note: on some occasions, U14-17s will be competing in Junior Carnivals more information on Cadet/Youth carnivals can be sourced via our contacting our office - lifesaving@coolumsurfclub.com.au.

Volunteer Positions

Each carnival our club is required to provide Carnival Officials and sometimes Water Safety. If you know of someone who can help out in these rolls please add their details to the form so our Carnival Organisers can allocate these positions as required.
